New Patient Registration

Download the registration form (DOCX, 612KB). You must complete at least pages 1 to 5 and sign page 5. Incomplete or unsigned forms will be rejected.

These forms are also available from reception.

Find your NHS number
You do not need to know your NHS number to use NHS services, but it can be useful to have it


How to Register

  1. Complete the form from the above link ensuring you provide as much of the information required as possible.
  2. Email the form to:
    • Field Road Surgery email: donccg.fieldroadreception@nhs.net
    • Northfield Surgery: syicb-doncaster.northfield-surgery-reception@nhs.net 
  3. We will arrange the transfer of your records once you have been accepted
  4. We will inform your previous practice that you are moving, but you can also let them know

Alternatively you can register by visiting the surgery and asking for a paper registration form or by registering using the online GP Registration Service.

Temporary Registration

If you are not registered with a GP but need to see one, you can get emergency treatment at any GP surgery. If you need treatment for more than 14 days, you will need to register with that GP surgery as a temporary or permanent patient.

You will need to complete a Temporary services form GMS3.

You can register as a temporary resident if you plan to live near the GP surgery for up to 3 months.

After 3 months you will have to apply to register with that surgery as a permanent resident.

You can register temporarily with a GP surgery while away from home for work, study or on holiday. You'll remain registered with your permanent GP surgery.

The temporary GP surgery will pass details of any treatment you have to your permanent GP surgery. They will add the information to your medical records.

If your application to become a temporary patient is refused, you can still receive emergency treatment for up to 14 days.
